Background Sepsis is an imbalance in host response induced by infection, which in turn leads to life threatening organ dysfunction, and is one of the leading causes of death in critically ill patients worldwide. Antibiotics cannot regulate host immune response, and powerful anti-inflammatory drugs (such as large doses of glucocorticoids) have risks of inducing secondary infection, aggravating metabolic disorder and the like, which highlight the limitation of the current treatment in the key link of regulating host response. Based on the theory of 'holistic view' and 'dialectical treatment', the traditional Chinese medicine shows unique potential in regulating complex inflammation and immune response by virtue of the multi-component, multi-target and multi-path synergistic action mode, and is regarded as a valuable supplementary strategy in comprehensive treatment of sepsis. Sepsis is mostly classified in the traditional Chinese medicine into the categories of warm disease, blood syndrome, syncope and the like, and the main clinical symptoms in the early stage of shock include chills and high fever secondary to suppurative infection, tachypnea with tachycardia, serious dyspnea and dying sensation, hypotension, constipation, reduced urine volume (oliguria), change of consciousness state, pale skin, damp coldness, change of pattern of spots or ecchymosis, multiple organ dysfunction and the like. At present, the treatment methods of clearing heat and detoxicating, activating blood and dissolving stasis, freeing viscera and purgating down, tonifying qi and relieving depletion are commonly used in clinic. However, the traditional Chinese medicine compound or research disclosed in the prior art focuses on the single pathogenesis of excessive heat toxin or incoordination between viscera, and the thinking of medication is often focused on the bitter and cold heat clearing or interior-dredging and purgation of big dosage. Although the scheme can relieve partial symptoms, a key pathogenesis is generally ignored, namely, the sepsis patient usually has a constitution foundation of 'yang qi deficiency and cold dampness easy to collapse' due to congenital deficiency or acquired imbalance. It is the internal cold-dampness and deficiency of healthy qi that causes the exterior-sensitive heat-toxin to sink deep and lodge with the interior-dampness, resulting in the problems of prolonged illness, difficult repair of tissue injury (easy ulceration) and rapid impairment of healthy qi (progress to shock). Clinically, the pre-shock patients show contradictory symptoms of shivering and high fever alternation, namely, damp-cold limbs, which is the external manifestation of cold-dampness stagnation transforming heat and vital qi deficiency evil loving complex pathogenesis.
