Abstract
The system for creating algorithmic clinical pans provides healthcare professionals with a specialized digital tool for composing individualized clinical plans without the need for programming. By means of dedicated graphical tools, it enables the creation of clinical plans for patient monitoring and self-management, which, before being assigned to patients, are simulated in the virtual application of a mobile device or other suitable terminal of the patient, exactly as the patient would receive them, in order to validate the smooth flow of the plan. The objective of the interconnected system is to simplify the process of creating clinical plans and to enhance remote communication and collaboration between healthcare professionals and patients/caregivers. The vital information generated and exchanged within the interconnected system meets the diverse expectations of stakeholders in healthcare, supporting personalized, preventive precision medicine.
