According to the 2023 statistics on registered persons with disabilities released by the Ministry of Health and Welfare, the number of registered persons with disabilities stood at approximately 2.64 million as of 2023 and is increasing every year. In particular, the proportion of registered persons with disabilities aged 65 or older is steadily rising. Those aged 65 or older number approximately 1.42 million, accounting for more than half of the total. This trend is expected to accelerate further due to rapid population aging. People with disabilities require continuous and regular treatment to prevent the worsening of their conditions and to improve them. However, they face difficulties in receiving timely and consistent treatment due to limitations such as the burden of medical expenses, mobility issues, and a shortage of rehabilitation facilities. The lack of rehabilitation facilities is considered such a major social issue that a new term, "rehabilitation refugees," has emerged to describe patients who are unable to receive rehabilitation services at a single hospital and are forced to move between multiple hospitals for treatment. To address the issue of rehabilitation refugees, the ‘Act on the Guarantee of Health Rights and Medical Accessibility for Persons with Disabilities’ was enacted in 2015, and the government is making various efforts, such as implementing the ‘Pilot Project for Primary Care Physicians for Persons with Disabilities’ in 2018. As one of the aforementioned government efforts, relevant laws and systems are being revised to provide home-based rehabilitation services to persons with disabilities. Although home-based rehabilitation services are being introduced, the issue of "rehabilitation refugees" has not yet been resolved. Existing home-based rehabilitation services are centered around government-run rehabilitation medical institutions, particularly those under the Ministry of Health and Welfare, making them difficult to access. More specifically, because services are provided primarily by these institutions, it is difficult to form home-based rehabilitation teams.
