Abstract

FIELD: medicine. SUBSTANCE: invention relates to an instrument for applying a medical liquid into the body of an animal or human. Furthermore, the invention relates to an apparatus for operating such an instrument, i.e., for controlling its operation, as well as to a method for creating a flow from two different fluids. The apparatus (11) for operating the instrument (10), intended for injecting a medical liquid into the body of an animal or human and comprising a flow line (17) for supplying the instrument (10) with a first fluid under pressure, a metering device (16) with a drive unit (25) for supplying a stored medical liquid as a second fluid, and flow channels (40, 47) for the first and second fluids, comprising a pressure accumulator (42) filled with the first fluid under pressure and connectable via the flow line (17) of the instrument (10) and a quick-acting valve (43) to the flow channel (40) of the instrument (10), and a control unit (44). The control unit (44) is configured such that for the injection of the medical liquid: to initiate delivery through the flow channel (40) of the instrument (10) of the first fluid from the pressure accumulator (42) by opening the quick-acting valve (43) to create an initial pulse (50, 51) of a first pressure, followed by supplying the first fluid under a second pressure (52) lower than the first pressure, and after the pulse (50, 51) of the first pressure, to activate the drive unit (25) of the metering device (16) of the instrument (10) to create a pulse (53) of a third pressure lower than the first pressure in order to deliver the second fluid through the flow channel (47) of the instrument (10). The instrument (10) for injecting a medical liquid into the body of an animal or human in interaction with the above-described apparatus (11) comprises a housing (12) provided with a handle (15) for holding and guiding the instrument (10), a shaft (13) extending in the distal direction and having a first flow channel (40) and a second flow channel (47), a flow line (17) provided for connection to the apparatus (11) which supplies the instrument (10) with the first fluid under pressure so as to supply the first fluid into the first flow channel (40), and a metering device (16). The metering device (16) has a receptacle (21) for a container (22) with the medical liquid as the second fluid and comprises a drive unit (25) for creating pressure in this container (22) and supplying the second fluid into the second flow channel (47). The metering device (16) also comprises an electrical line (18) connectable to the control unit (44) of the apparatus (11) for controlling the drive unit (25). The method for creating a flow from two different fluids is characterised in that the first fluid contained in the pressure accumulator (42) under a first pressure is introduced into the flow channel (40) by means of the quick-acting valve (43). After opening the valve (43), the second fluid contained in the container (22) without excess pressure is introduced into the flow channel (47) under a pressure lower than the first pressure by reducing the volume of the container (22). To reduce the volume of the container (22), the drive unit (25) with a servomotor is used, and a common control unit (44) is used to control the drive unit (25) and the valve (43). Gentle administration of the medical liquid is also achieved by the fact that the first fluid is supplied under a higher pressure than the second fluid. EFFECT: reduction of dead volumes and loss of medical liquid, facilitating the preparation of the instrument for use with different types of medical liquids due to the possibility of placing the container with the medical liquid in the receptacle of the instrument by arranging the supply of the injectable medical liquid in the instrument. 19 cl, 6 dwg

Description

The present invention relates to an instrument for administering (applying) a medical fluid to an animal or human body. Furthermore, the invention relates to apparatus for operating such an instrument, i.e., for controlling its operation, as well as to a method for creating a flow of two different fluids. For example, for needle-free injection of drugs or other medical fluids into the tissue of human or animal bodies, devices are used in which a fluid under high pressure is used to pierce the patient's skin and create a needle-free puncture channel into which a medical fluid under lower pressure is then injected. Publication EP 3015076 A1 discloses an applicator that can be used to spray a medical fluid, such as a hemostatic fluid, using compressed air or another pressurized gas, thereby applying it to the surfaces to be treated. The applicator comprises a holder with a flow channel connected to a gas pressure source, containing a valve. The flow channel terminates in a spray nozzle. A standard syringe can be attached to the applicator. The user, holding the applicator, can open the air channel valve with a finger and actuate the syringe plunger with a thumb, directing the liquid contained in the syringe to the spray nozzle, where it is released as a spray. Furthermore, US 2013/0102957 A1 discloses an instrument for the needle-free injection of a substance into the body. The instrument has a reservoir for holding the injectable substance, connected via a flow channel to an injection nozzle. To eject the medical fluid in a short, sharp jet, an electrodynamic drive is connected to a piston located in the reservoir. This drive creates a change in fluid pressure with a very steep leading edge at the injection nozzle. This sharp increase in pressure generates a sharp jet of liquid capable of piercing skin. After this sharp increase, the pressure drops to a low level to inject the fluid into the tissue, and then, upon completion of the operating cycle, the pressure returns to zero. Furthermore, for needle-free injection of medical fluids, publication EP 3714926 A1 proposes a tool with multiple flow channels delivering various fluids to the injection nozzle. Specifically, it features a central channel for delivering the injection medium, which creates a puncture channel, and a second channel surrounding it, through which the medical fluid is delivered to the injection nozzle. The known instrument is, in principle, well suited for use on patients. However, the channel provided for the delivery of the therapeutic fluid must be completely filled with it. If a patient-specific therapeutic fluid is used, the amount of therapeutic fluid expended will be greater than the amount of fluid administered to the patient. The same applies to the system described in EP 3040101 B1, which describes an instrument with two flow channels. The first channel directs the injection fluid to the applicator nozzle, while the second channel directs the therapeutic fluid. The instrument is powered by a device with a pressure accumulator and several valves, used, in particular, to supply the injection fluid to the instrument at a predetermined pressure change rate. The invention was based on the task of developing an improved solution for needle-free administration of therapeutic fluids. This problem is solved in the apparatus according to claim 1, in the instrument according to claim 8, and also in the method according to claim 19 of the invention formula. The instrument according to the invention is used for injecting a medical fluid into the body of an animal or a human in cooperation with the apparatus according to the invention, wherein the instrument is configured to administer (apply) two different fluids. The first fluid serves to needlelessly form or open a channel (puncture channel) in the tissue being treated or affected, and is delivered with a pressure pulse characterized by a steep pressure increase at its leading edge. The instrument also comprises a dosing device having a container for a container with a medical fluid as a second fluid. The medical fluid may be, in particular, a therapeutic (used to affect the body) fluid, such as a medicinal fluid, a cell suspension, or any other fluid that is used to treat the human or animal body or to affect human or animal tissue. The dosing device comprises a drive unit, by means of which pressure can be created in the container or the volume of the container can be purposefully reduced. For example, the drive unit can specifically act on the container to reduce its volume to smoothly displace the second fluid. When pressure is created in the container, the second fluid is fed into the second flow channel. The dosing device also includes an electrical line connected to the apparatus' control unit to control the drive unit. Since the aforementioned container is located in the dosing device directly next to the instrument, dead volumes are minimized. This prevents loss of therapeutic fluid
