RU-2861511-C1 - METHOD FOR TREATING INFLAMMATORY PROCESSES OF PERIODONTAL TISSUES
Abstract
FIELD: medicine; dentistry. SUBSTANCE: invention can be used for treating inflammatory periodontal diseases. Anamnesis is collected, hygiene indices and the condition of periodontal pockets are determined. An express collection of volatile organic compounds (VOCs) from periodontal tissues is carried out by a 10-second application of a hygroscopic material, followed by sensory diagnostics using a piezoresonance device and a point assessment of the indicators. Rinsing with yarrow hydrolate and repeated VOC diagnostics are performed to assess the primary dynamics. If inflammation decreases, a course of independent rinsing with yarrow hydrolate is prescribed for 10 days, after which a control VOC collection and sensory diagnostics are performed. Based on a comparative analysis of the data before and after using the hydrolate, a decision is made on professional cleaning and treatment in accordance with the identified degree of inflammation. EFFECT: timely relief of the inflammatory process and reduction of the risk of complications by using highly sensitive sensory diagnostics of VOCs in combination with the anti-inflammatory effect of yarrow hydrolate, which increases the accuracy of assessing the dynamics of the condition of periodontal tissues. 1 cl, 2 dwg, 2 tbl, 2 ex

Description
The invention relates to medicine, namely to dentistry, and can be used to diagnose inflammatory processes in the oral cavity, in particular to methods for detecting inflammatory processes in periodontal tissues using the analysis of volatile organic compounds (VOCs) emitted by the oral mucosa. Inflammatory processes in the oral cavity are one of the most common pathologies in dentistry. According to the World Health Organization, about 50% of the adult population suffers from various forms of periodontitis. Some studies report that the prevalence can reach 70-80% in people over 65 years of age [Chapple I.L.K., Mealy B.L., Van Dyke T.E., Barthold P.M., Dommisch H., Eickholz P., et al. Periodontal health and gingival diseases and conditions in the intact and reduced periodontium: report of Working Group 1 of the 2017 World Workshop on the Classification of Periodontal and Peri-implant Diseases and Conditions. J Clin Periodontol. 2018;45(Suppl 20):S68–s77.]. Diagnosis of inflammatory periodontal diseases can be difficult due to the sluggish inflammatory process in the early stages of the disease or vague symptoms of the disease, such as mild bleeding or slight tooth mobility [Helal O, Goestemeyer G, Kroys J, Fawzi El Sayed K, Gratz K, Schwendicke F. Risk factors for tooth loss in patients with periodontitis: a systematic review and meta-analysis. J Clin Periodontol. 2019;46:699–712]. The importance of rapid and accurate diagnosis of inflammatory periodontal diseases is essential to prevent disease progression, reduce the risk of tooth loss and improve overall health, as periodontitis is associated with systemic diseases such as diabetes mellitus and cardiovascular disease [http://r967198x.beget.tech/wp-content/uploads/2022/10/Obsledovanie_pazientov_s_boleznyami_parodonta_2edition_Yanushevich_i_dr.pdf]. Existing clinical and laboratory methods for diagnosing the dynamics of inflammatory diseases in the oral cavity are carried out, among other things, by calculating hygiene indices (PMA, GI, PI), determined in accordance with the Clinical Guidelines "Chronic Periodontitis" (K05.3), approved by Resolution No. 1 of the Council of the Association of Public Associations "Dental Association of Russia" dated December 20, 2024. Hydrolates, 100% natural therapeutic distillates from freshly picked plants, have antimicrobial and antiviral activity and are used for oral care. Stimulation of the mucous membrane with hydrolate is performed in cases of aseptic inflammation of periodontal tissues; it is an effective anti-inflammatory method due to which local immunity is activated, which helps reduce swelling and hyperemia of tissues, relieve pain and improve healing; there is a decrease in the bacterial load due to the antibacterial properties of the hydrolate [Yakovleva G.P. Medicinal plant materials. Pharmacognosy: a study guide // G.P. Yakovleva K.F. Blinova. St. Petersburg: Spetslit, 2010. P. 133]. Yarrow hydrosol (Achillea millefolium) is an aromatic water obtained by steam distillation from the aerial parts of the yarrow plant. It contains biologically active components such as flavonoids, essential oils, and polysaccharides, which have anti-inflammatory, antimicrobial, and healing properties. It has an antiseptic effect, reducing the number of pathogenic microorganisms in the oral cavity. It promotes the healing of mucous membranes and periodontal tissues. It is used as a mouthwash for gingivitis, periodontitis, and other inflammatory diseases of the oral cavity. It has a mild soothing effect and reduces discomfort and inflammation. (Imtiaz S., Gopalakrishnan B., Venkatesalu V.//. Pharmacognosy, phytochemistry and pharmacological properties of Achillea millefolium L.: A review. "Phytotherapy Research" 2017. Medicine, Biology DOI: 10.1002/ptr.5840, 20465953). Existing methods of sensory diagnostics of the dynamics of inflammatory diseases in the oral cavity mainly consist of minimally invasive and labor-intensive methods. Sensor diagnostic devices are designed to detect, recognize, and analyze volatile organic compounds (VOCs). They utilize sensor arrays that mimic the human olfactory system and enable rapid and accurate determination of the composition of air or other substances. Piezoelectric sensors are a type of sensor that utilize changes in the resonant characteristics of piezoelectric materials to detect substances. They operate by changing the resonance frequency or oscillation amplitude upon interaction with volatile organic compounds (VOCs). Features of piezoelectric sensors: - high sensitivity to masses, even in very small quantities; - quick response; - the ability to work in real time; - high selectivity when using special coatings or surface modifications (https://vestnik.rsreu.ru/ru/archive/2023/vypusk-85/1425-1995-4565-2023-85-158-169?ysclid=md4llo3pqu75662682). A piezoelectric resonant sensor consists of a piezoelectric crystal or film that is excited in resonant mode. When VOCs interact with the senso
